"Compliance with Hand Hygiene in Emergency Medical Services: An International Observational Study"

Hand hygiene is a cornerstone of infection prevention and control in healthcare settings, including emergency medical services (EMS). Despite its critical importance, compliance with hand hygiene guidelines remains suboptimal. To understand the global landscape of hand hygiene compliance in EMS, an international observational study was conducted, yielding valuable insights into current practices and areas for improvement.
Study Design and Methodology
The study, published in the journal Prehospital Emergency Care, involved 1,692 EMS providers across 11 countries. It employed a prospective, observational design, focusing on hand hygiene opportunities and compliance during patient care encounters. The World Health Organization's (WHO) 'Five Moments' for hand hygiene were used as the gold standard for determining hand hygiene opportunities.
Key Findings
Overall Compliance: The overall hand hygiene compliance rate was 60.2%, ranging from 34.5% in low- and middle-income countries to 79.2% in high-income countries.
Compliance by Moment: Compliance was highest before touching a patient (74.3%) and after touching a patient (68.7%), but lowest before performing an aseptic task (49.2%) and after exposure to body fluids (52.1%).
Factors Affecting Compliance: Factors associated with higher compliance included working in a high-income country, having a higher level of education, and being a paramedic or emergency physician.
Regional Variations and Challenges
The study highlighted significant regional variations in hand hygiene compliance. High-income countries had higher compliance rates, likely due to better resources, training, and awareness. However, even in these countries, compliance was far from perfect, indicating that no region is immune to the challenge of ensuring consistent hand hygiene.

Barriers to Compliance
Several barriers to hand hygiene compliance were identified, including lack of access to soap and water or alcohol-based hand sanitizers, lack of time, and forgetfulness. These findings underscore the need for multi-faceted interventions to improve compliance, such as ensuring adequate supplies, providing reminders, and offering training and education.
Implications and Recommendations
The findings of this study have important implications for EMS systems worldwide. They highlight the need for targeted interventions to improve hand hygiene compliance, particularly in low- and middle-income countries. Such interventions could include enhanced training, provision of adequate supplies, and the implementation of systems to remind and enable hand hygiene.

Moreover, the study underscores the importance of international collaboration in infection prevention and control. By sharing best practices and learning from one another, EMS systems can work together to improve patient safety and reduce healthcare-associated infections globally.
